National Committee for Quality Management
Complaints
Submission requirements and process
Scope
Complaints concern service, conduct or institutional practice within the Committee's responsibility. Describe the specific matter and the outcome you seek.
Choose the right procedure
Publication errors use corrections. Eligible challenges to a decision use appeals. Suspected false representation uses misrepresentation reporting.
Channels
Submit through the designated form or contact secretariat@ncqm.org to arrange another accessible route. Membership or payment is not required to raise a concern.
Information
Identify the activity, relevant dates and people only as necessary. Provide evidence you are authorized to share and identify confidentiality needs.
Acknowledgment
A receipt provides a reference. The handling function confirms the matter's scope, next step and any information needed to proceed.
Review
The matter is assigned for review with relevant interests considered. A person does not act as the sole reviewer of a complaint about their own conduct.
Outcome
The response explains the disposition and corrective action that can appropriately be disclosed. Private third-party information remains protected.
Progress
If more information or time is needed, the complainant is informed through the agreed contact route. Applicable legal requirements remain controlling.
Submit and follow up
Select Submit complaint after reviewing the information. Keep the reference for secure follow-up and any available review route.
