Evidence Review Worksheet

Version 1.0 · Tool & checklist

Use this worksheet to organize a review of technical evidence before drawing a conclusion. Retain the completed record with the source and decision it supports.

Define the review

Record the decision, requirement and object first. This avoids treating a document as sufficient merely because it appears formal or contains a favorable result.

Review item Information to record
Decision The question and choices the evidence must support
Requirement Document, edition, clause or agreed condition
Object Exact product, configuration, process or population
Source identity Issuer, document title, identifier and edition
Evidence form Full report, excerpt, data, declaration or interpretation
Sample What was examined and its relationship to the object
Method Designation, edition and relevant procedure
Conditions Operating conditions, environment and reporting basis
Result Value or conclusion as originally expressed, including units and qualifiers
Uncertainty Reported uncertainty and its relevance to the decision
Completeness Missing pages, references, qualifications or supporting material
Currency Changes that could affect relevance, not merely document age
Rights Permission to access, retain and share the material
Reviewer Person or role responsible for the review
Open issues Questions, evidence needed, owner and follow-up
Conclusion What is supported, conditions and unresolved limits

Use the worksheet

Distinguish a checked fact from an assumption. For an unresolved item, record why it is unknown and whether the uncertainty could affect the decision.

Do not convert the number of completed fields into a quality score. A single missing condition can be decisive, while a fully completed record can still support only a limited conclusion.

Review and maintenance

Have the appropriate technical person examine material gaps. Keep the source edition and reasoning with the final decision. Revisit the record when the object, requirement or evidence changes.
